Team Approval Applications

The 2024 Team Approval Application is now open. Applications are due July 1, 2024.

When completing the application, be sure to read ALL instructions provided.

All applying and renewing teams must submit a non-refundable initial application fee of $335. Payment may be submitted by check, Visa, MasterCard, Discover or American Express. If a team is approved, this fee will count as the team’s first year listing fee. Team fees cover the administration of the ACPA Team Approval program. 

Annual Self-Audit Reports and Listing Fees

The 2025 Self-Audit Report will open in August 2024. 

The 2025 listing fee is $220. Pay the fee online by logging into your team's account and selecting "ACPA Store" from the menu. Please be sure to pay using your TEAM account not your individual account.

All ACPA Approved Teams must submit a Self-Audit Report and pay the team listing fee in the fall of each year. The purpose of the self-audit report is to help both the team and the Commission ensure that the team remains in compliance with the Standards for Approval of Cleft Palate and Craniofacial Teams during its approval period. The self-audit report can also be used as a tool to support teams as they track goals, quality assurance and performance

The self-audit report process is simpler than the full application and is only evaluated by the Commission if there is missing information or a potential issue. If an Approved Team does not complete the self-audit report or pay the team listing fee by the set deadline, the team may lose its approval status. Team fees cover the administration of the ACPA Team Approval program.
